Job Summary and Qualifications
The Transplant Social Worker III is responsible for assessing the psychological, social, cultural and / or financial needs that impact transplant patients.
Your role has the ability to utilize social services, counseling and clinical knowledge in the assessment of these needs.
You are knowledgeable in ESRD / transplantation Medicare guidelines.
You are responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources.
Your role serves as a transplant social worker resource though out the facility.
You have knowledge of payer source implications on plan of care and resources to maximize patient’s successful outcome.
What qualifications you will need :
Licensure / Certification / Registration : Licensed Clinical Social Worker (LCSW) in the state of Colorado.
Education : Master of Social Work (MSW) from an accredited school of social work required.
Experience : 3-5 years recent clinical social work experience in transplantation. 5-7 years recent clinical social work in an acute setting.
Special Qualifications : Must have excellent written, verbal and interpersonal communication skills and able to work with highly diverse population.
Must be knowledgeable of State and JCAHO standards. Must be knowledgeable of ESRD / transplantation Medicare guidelines.
Must have ability to perform multiple tasks simultaneously and work in high stress environment. Must be able to apply critical thinking and formulate diagnostic impressions based upon review of systems, history, symptoms and current presentation.
Must be able to provide crisis intervention and utilize system resources / referrals.
Degree of supervision required : Involves general guidance and direction by the Department Director. Employee will be expected to perform all job duties independently and in accordance with established departmental and hospital policies and procedures.
This role requires you to be fully vaccinated for COVID-19 based on local, state and / or federal law or regulations (unless a medical or religious exemption is approved).
