Social Worker
Job Summary :
Provide support and assistance to families who have diabetic children that may be facing a variety of challenges.
Key Responsibilities :
- Provides psychosocial support / intervention and care to newly diagnosed and established children with type 1 and type 2 diabetes.
- Provides the mental health assessment component in interdisciplinary care of patients and makes appropriate referrals for treatment.
- Does phone consultations with patients, other medical professionals, and protective services / Court officials to advocate around medical care of diabetes.
- A combination of previous mental health clinical experience and understanding of ethnic and economic diversity and its impact on management of chronic medical conditions is essential.
- Excellent interpersonal and communication skills are required.
- Demonstrated ability to work collaboratively is required as well as the ability to plan strategically, set priorities, and establish and achieve agreed clinic objectives.
This person must be flexible and able to adapt to changing circumstances.
- Provides ongoing education to the community, medical professionals and students.
- Contributes to new and ongoing diabetes clinical research protocols.
- Should have basic computer skills for charting / data entry of patient contacts.
- Maintains high standards of existing diabetes program
- Maintains credentials for licensure as a clinical social worker (LCSW) and, eventually as a Certified Diabetes Educator, CDE.
- Experience using evidenced based interventional in the treatment of co-morbid mental health conditions.

The Pediatric Clinic offers complete education and support for children and adolescents with type 1 diabetes. The expectation is that by the time patients are ready to leave home for college or the workforce, they will have the ability to manage their own blood sugars.
The clinic is certified by the American Diabetes Association, and our nurses, dietitians, nurse practitioners, physician assistants and social workers are Certified Diabetes Educators.
Our pediatric endocrinologists also participate in studies that strive to advance and improve glucose monitoring and closed loop technology through observational studies and clinical trials at our Center.

Qualifications :
Minimum Qualifications :
Applicants must meet minimum qualifications at the time of hire.
- Master’s degree in Clinical Social Work (MSW)
- Clinical License (LCSW) OR eligible for clinical license within 6 months to practice in Colorado.
Preferred Qualifications :
- Experience working with pediatric populations
- Fluency in Spanish
- Knowledge of diabetes or other chronic medical condition and their impact on children and families.
Knowledge, Skills, and Abilities
- Case management / medical case management background
- Experience managing complex caseloads
- Strong understanding and experience with assessing mental health needs, substance use and effects on ability to access care
- Knowledge of child protection issues
- Demonstrate fluency and clarity in written communication
- Display clear, personally sensitive, and culturally competent verbal communication
- Ability to work collaboratively within a multidisciplinary team
- Show strong organizational skills and the capacity to coordinate multi-site activities

Anticipated Pay Range :
The starting salary range (or hiring range) for this position has been established as HIRING RANGE : $51,000-$71,000.
The above hiring range represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting.
This position is not eligible for overtime compensation unless it is non-exempt.
