Provides information to patients regarding available transportation. Participates in appropriate research activities approved by Courland Dialysis. Participates with multidisciplinary team to facilitate patient’s adherence to their plans of care. Promotes a constructive approach to problem solving within the facility and takes an active role in QA / CQI process. Utilizes capabilities of the computer systems. Educates and counsels patients regarding Courtland Dialysis Directives and provides follow up per policy for Advance Medical Directives. Coordinates social work in-service education for health professionals. May provide in-service education for community organizations if requested with the approval of the Director of Nursing. Participates in and promotes team concept throughout all clinical activities in accordance with policies and as allowed by state regulations. Qualifications Master of Social Work with specialization in clinical practice from a college or university accredited by the Council of Social Work Education; and must be a licensed or certified Social Worker as required by individual state law. Prefer at least one year experience and knowledge of ESRD.

The Child and Adolescent Day Hospital is a clinically intensive day program for children (ages 5-12) and adolescents (ages 13-18) with serious psychiatric problems. Designed as an alternative to inpatient and residential care, it provides rapid assessment and crisis stabilization, and the structure and intensity of an inpatient program but for patients who do not require 24-hour treatment. Services provided include Structured Therapeutic Group Treatment; Family Meetings conducted by a licensed clinical professional (LCSW-C or LCPC) and Psychiatric Evaluation and Medication Management provided by a staff psychiatrist. Additional groups are provided by visiting artists from September thru May offering Drumming, Movement, Art and Drama therapies. About Us : The Baltimore Academy has established itself as a premier provider of Full Day Special Education services for students in grades 6 through 12. The 12 month school program serves male and female adolescents ages 12 through 21 who are exhibiting serious emotional and behavior disorders and who are not yet able to function in less restrictive school settings. We are looking for a Full-Time LGSW or LCSW-C, with experience working with children for our special education program.Responsibilities : The overall responsibility of the school Social Worker is to manage all cases of students enrolled in The Baltimore Academy and to assure provision of all social work and counseling services listed on the students, IEPs.Requirements : Licensed LGSW is required to provide counseling services to youth grades 6-12 in a school setting. Work also requires regular consultation with school staff.Minimum Requirements : Licensed LGSW Master’s degree 0-1 years of experiencePreferred area of college / graduate concentration : Master’s Degree in Social Work preferred Prefer 1 year experience working with adolescents in a psycho educational setting.
