Facilitate discharge planning across a multidisciplinary care continuum that includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan. Works collaboratively with the multidisciplinary staff to design and implement treatment plans and discharge plans for the patient. Be able to provide appropriate referrals depending on the needs of the patient. Be able to assist with financial issues. Facilitate relaxation exercises and assist patients and family members in achieving positive coping strategies. Provide education and counseling to patients and families around issues related to adaptation to the patient's illness and / or life situation. May utilize crisis intervention, brief and long-term individual, group, and family therapies. Develop and facilitate support groups or special functions. Address and facilitate advance directives and able to assist patient / family with completion.
