Job Summary :
The Community Health Worker (CHW) Project Director position will be responsible for managing and coordinating projects and initiatives related to Community Health Workers for the Client Department of Health (TDH), Division of Health Disparities Elimination.
Must live in the State of Tennessee.
Key Duties / Responsibilities :
Provide coordination of CHW initiatives across the division and the department
Plan and convene meetings with internal and external partners that support, hire, or train CHWs
Serve as a subject matter expert on CHW infrastructure sustainability efforts and best practices
Strengthen community-based organization capacity as health system partners and CHW employers
Leverage existing resources and establish new partnerships that support the CHW workforce in Tennessee
Support collaboration and strategic planning that integrate and align CHW efforts into broader statewide workforce efforts

Education and Experience :
Bachelor’s Degree in Social Work, Human Services, Healthcare, Public Health, or related field. Four (4) years of relevant community health worker, community outreach, orhealth education experience may be considered in lieu of degree.
Two (2) years of prior experience in a CHW role; supervisory experience is a plus
Three (3) years of experience in community outreach, health education, or in
a role representing or supporting local community agencies
One (1) year of progressive leadership experience relevant to a Population
Health role is preferred
Two (2) years of prior experience working in a Community Health Worker or
Social Work model preferred
