Social Worker Job Description :
Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
Provide information to the resident, and / or family member, on residents' rights and responsibilities
Qualifications :
Must have CSW or LCSW
Must have experience as a Social Worker
Must have long term care experience
Must be a self-starter and energetic
Strong Leadership & Management Skills
Strong verbal & written communication skills

Work with Admissions department to meet short term and long-range census goals
Develop and provide information for marketing materials or presentations as needed.
Help manage the admissions process to ensure satisfaction for residents and families
Create, manage and monitor new business development plans
Conduct research as needed.
Qualifications of the Marketing Liaison :
Must have marketing experience
Good communication skills
Will hold events and build up relationships within the community
Must have long term care experience
Must have connections with local hospitals

Conduct and coordinate the development and completion of the resident assessment (MDS)
Maintain and periodically update written policies and procedures that implement MDS and care plan.
Assist the resident in completing the care plan portion of the resident’s discharge plan.
Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents.
Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings.
Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents.
Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews.
Qualifications :
Must have current New York RN License
Must have experience as an MDS Coordinator in long term care
Must have excellent leadership skills
Strong clinical background and knowledge
