The Forest Home Clinic is a new Family Medicine Physician Residency Clinic located on Milwaukee’s Southside. This location will begin seeing patients July 1, 2023 with 6 residents and 6 faculty physicians. This is a new strategic location for Froedtert & the Medical College of Wisconsin with the intent of developing a culture of quality medical education in urban community settings and to address health inequities in Milwaukee’s Southside. This job is ON - SITE. FTE : 0.500000 Shift : 1 Working 40 hours per 2 week pay period during location hours of Monday-Friday 8am-6pm Job Summary : In accordance with established policies and procedures, implements and facilitates discharge planning. Provides social services assessment and interventions for illness-related situations for patients and their families across the care continuum. Other duties as assigned. EXPERIENCE PREFERRED : A minimum of 5 years of experience in acute care setting is preferred. EDUCATION DESCRIPTION : Bachelor's degree in Social Work is required. SPECIAL SKILLS DESCRIPTION : Analytical skills necessary to assess patients needs, develop associated discharge planning and provide sound advice and guidance. LICENSURE DESCRIPTION : Current State of Wisconsin license in social work is required.
